The Nine Admission Requirements for Adolescent Males Entering Our Residential Treatment Program
Every referral is reviewed against the same nine points. They cover clinical fit, age, motivation and the documentation that has to be in place before a young man can move in.
One
The need for further treatment is the first requirement for admission.
Two
The applicant must meet the criteria for chemical abuse or dependency under the ASAM placement criteria.
Three
The length of stay is variable, depending on the resident’s progress.
Four
The applicant must exhibit motivation to continue in the recovery process.
Five
Endeavor Place, LLC serves adolescent males between 14 and 19 years of age.
Six
We require a copy of the Rule 25 Assessment and Placement Summary.
Seven
A Client Placement Authorization must be signed and in place before admittance, or approval from insurance.
Eight
Admission hours are 8am to 4pm, Monday to Friday, at 21539 120th Street, Verndale, MN 56481.
Nine
A licensed counselor or the program director processes the intake, and a CD technician may complete the process.
Paperwork Required Before Admission to Our Verndale Residential Treatment Facility
Two documents have to be in hand before a young man arrives. Missing either one is the most common reason an admission gets delayed.

Admission Hours and Where to Arrive on the Day of Intake
Admission Hours
8am to 4pm, Monday through Friday. Admissions are not taken on weekends.

Who Handles Intake
A licensed counselor or the program director, with a CD technician possibly completing the process.
The facility itself operates 24 hours a day, seven days a week. Admissions are a weekday daytime process because intake needs a licensed counselor or the program director available to do it properly.
How Motivation and Clinical Need Are Assessed Before a Young Man Is Accepted
The first criterion is need for further treatment, and it sits first for a reason. If a young man does not clinically need residential care, this is not the right placement regardless of how convenient it may be for the people arranging it.
Motivation is judged reasonably, not strictly. Very few adolescents arrive keen. What we look for is some willingness to keep going with recovery, not enthusiasm.
Length of stay is not fixed at intake. It depends on progress, and we won’t quote a number of days we can’t stand behind.
Referral Sources We Accept for Adolescent Residential Substance Abuse Treatment
We take referrals from across all counties in Minnesota:
- Probation officers
- County and tribal CCDTF coordinators and Rule 25 assessors
- Social workers
- Inpatient and outpatient treatment programs
- Detention facilities
- Voluntary and family placement
Common Reasons an Admission Is Delayed or a Referral Is Not Accepted
We’d rather say no early than discharge someone in week three because the setting was never right. The usual sticking points:

| Safety or medical concerns | Some presentations cannot be safely held in an unlocked eight-bed house. |
| No beds available | Capacity is eight. When full, referrals go on a waiting list. |
Questions Referring Workers and Families Ask About Admission
Can a young man be admitted the same day?
Only if the paperwork is already complete and it’s a weekday between 8am and 4pm. Otherwise, no.
Does a court order guarantee admission?
No. A referral still has to meet the clinical criteria and be a safe fit for the setting.
Does a resident need to be enrolled in school?
Yes. Ability to enroll is part of who this program serves, and residents attend school during their stay.
Are families involved from the start?
Wherever possible. Parents and guardians are part of the process from intake onward.
